New Patients
Click the link to complete the secure online intake form. Basic demographics and copies of both your driver's license/ID and primary/secondary insurance (if applicable) are required.

After review and approval (typically 24-48 business hrs), you will receive an email with a link to schedule an appointment.
**Please select NEW PATIENT Visit when choosing an appointment type/time to allow available options (Telehealth or In-Office)

Current Patients
Online scheduling is available via your patient portal. If you have not set up your portal account, please visit the portal link and select "register." If you need additional assistance, please review the Patient Portal Help Guide or contact us for assistance!

**Please choose the correct appointment type as there are time restrictions allocated to each visit reason. Selection of the wrong visit type will limit your appointment to the allotted time slot. Please see below options:

30 Minutes: Chronic/Follow-up Visit (Telehealth or In-Office)​

15 Minutes: Sick/Problem Visit (Telehealth or In-Office)

20 Minutes: FMLA/Disability/Misc Letters (Telehealth or In-Office)

60 Minutes: Medicare Annual Wellness (Telehealth or In-Office)

40 Minutes: Annual Wellness Visit Male or Female w/o GYN (In-office)

40 Minutes: Well-Woman Exam w/ Gyn (In-Office)​
